To make an appointment, please call the above number Monday through Friday from 8:00 am to 5:00 pm.
When calling to make your first appointment, please have the following information at the ready:
- Referring physician name and office telephone
- Patient name, address, contact phone, Social Security number
- Insurance provider and account number
- Primary diagnosis (the reason for the appointment)
- Patient medical records
These forms are required for the first visit. Please print these forms and bring them with you as they cannot be emailed or faxed to the Katmai Oncology Group office.
You may complete these forms at the time of your appointment. In this instance, please arrive 15 – 20 minutes early to give yourself time to complete them. Please review all information carefully.
Additionally, take inventory of all current medications – both prescription drug and over-the-counter (OTC) medicines, vitamins and/or dietary supplements. This information plays a crucial role in determining treatment, so please be as thorough and complete as possible.
