The First Steps
- These forms are required for the first visit. Please print these forms and bring them with you as they cannot be emailed or faxed to the Katmai Oncology Group office.

- You may complete these forms at the time of your appointment. In this instance, please arrive 15 – 20 minutes early to give yourself time to complete them. Please review all information carefully.

- Additionally, take inventory of all current medications – both prescription drug and over-the-counter (OTC) medicines, vitamins and/or dietary supplements. This information plays a crucial role in determining treatment, so please be as thorough and complete as possible.
