Making an Appointment

To make an appointment, please call the above number Monday through Friday from 8 am to 5 pm. (Although we often are closed between 12 and 1, please try again.)
When you call, you will need the following information:
- Your referring physician’s name and office telephone
- Your name, address, contact telephone, social security number, insurance provider and account number;
- Primary diagnosis (why you need to be seen)
Your medical records will need to be received by our office at least 24 hours prior to your appointment. They can be hand delivered, mailed, or faxed to the number above. Please make the appropriate arrangements with your referring physician’s office.

Please print the forms and bring them along to your visit. They cannot be e-mailed to our office, you must print them and bring them along. You can, of course, complete the forms at the time of your appointment. If you wish to do that, please come a few minutes early so you will have time to fill out the forms. Please review them to ensure you have all the information needed.
You need to let us know all the current medications your are taking, both prescription drugs as well as over the counter medicines and vitamins or dietary supplements.
